Billing Information

We accept all forms of insurance with the exception of Health Springs. For billing questions, please call our office at 251-333-3333 and we will guide you as to what documents to bring to your next visit.

Payment Policies:
• All copays are due at the time of service.
• A minimum of 50% of deductible is due at the time of service for testing and procedures.
• Same day cancellation fees for testing apply.
